Graphical artist deliverables

Hello, i will soon order a project from a graphical artist. Beside the .psd or whatever format he will produce the design, i should ask him a document/declaration where he states that he gives us complete rights to use his work?

Is there around a template for such a declaration?

Google "transfer of copyright ownership form" dude. That's its official name.

Make sure if the artist used any fonts that you get a copy of those fonts. When we have our stuff designed we always request the artist use free fonts that they can distribute. It really sucks to try and change a menu or something down the road when you don't have the correct font.
